A 50-year-old woman presented with long-standing left corneal opacity resulting in diminished visual acuity. B Optical coherence tomography revealed focal hyperreflectivity in the mid and deep corneal stroma, corresponding to the lipid deposits seen clinically, with sparing of the epithelium and anterior stroma. A penetrating keratoplasty was performed. C Hematoxylin and eosin staining demonstrated focal pronounced mid and deep stromal neovascularization, scarring and foamy cells the anterior stroma was spared x20. D Immunostaining for adipophilin accentuated the numerous foamy cells, highlighting lipid-containing vesicles x60 and inset.

We find this case to be interesting for a several reasons. First, the presentation of solitary metastasis to the anterior chamber angle secondary to prostate adenocarcinoma is highly unusual. Second, from a management standpoint, this patient did not require invasive intraocular biopsy in order to establish diagnosis. Third, the ocular metastasis responded completely to androgen deprivation therapy rather than the conventional radiotherapy techniques frequently used to treat intraocular tumors and his vision was fully restored. Finally, this case required collaboration between a large number of subspecialty services, and consequently we feel it will appeal to a wide audience.

Retinochoroidal ColobomaRCC is a developmental anomaly that occurs due to failed or incomplete closure of the embryonic fissure. Associated complications include Pre-senile cataract , Retinal detachment, and rarely Choroidal neovascularisationCNV. Prophylactic Barrage laser around the coloboma helps in prevention of retinal detachment in these eyes. CNV in these eyes have been reported to respond well to Ant-VEGF therapy.
