Post-traumatic pseudophacocele

A 65-year-old man presented at the Dr. Rajendra Prasad Centre for Ophthalmic Sciences with diminished vision in his right eye following a road traffic accident 2 months earlier. On examination, visual acuity in the right eye was hand motions close to the face, with accurate projection of rays in all four quadrants. Slit lamp examination revealed superior subconjunctival migration of the intraocular lens, with diffuse illumination revealing a pseudophacocele (A). Anterior segment optical coherence tomography (AS-OCT) further helped to access the integrity of the underlying sclera and exact location of the intraocular lens, with a subconjunctival, convex, hypodense space formed by the intraocular lens (IOL) migration (B). Removal of the IOL was performed subconjunctivally, and the scleral perforation was repaired. The patient’s best-corrected visual acuity postoperatively was 20/200.
