The patient is a 52-year-old man with a complaint of a red, painful, bulging right eye with double vision and blurring for 2 weeks. He has had several similar, but less severe, episodes in the past 9 years. It occurs more commonly with the right eye, but sometimes occurs with the left eye. He has a few episodes each year lasting a few weeks each and treats himself with ibuprofen. The current episode is more severe than usual and has not responded to ibuprofen. In some cases he has concurrent cold symptoms, but not during the current episode.
